    I’ve been using the plugin for a couple months fine, but all of a sudden, it doesn’t even work anymore, meaning someone can submit the form without even entering an answer to the equation. It doesn’t validate.

    I haven’t changed anything in my site nor the plugin. I’m using WP 4.1 and math captcha 1.2.4

    Me too. Same issue, but the WP Admin does require it, if set.
